Postpartum

The Fourth Trimester

New mother resting in bed with her sleeping newborn on her chest

The weeks after birth are often called the fourth trimester. Your body is healing, your baby is adjusting to the world, and your whole life is reorganizing. You deserve as much care now as you did in pregnancy.

How long does recovery take?

The postpartum period generally lasts 6 to 8 weeks, though recovery varies from person to person (NCBI). The American College of Obstetricians and Gynecologists recommends that postpartum care be an ongoing process, not a single visit, with contact with your provider within the first three weeks (ACOG).

What is normal

  • Bleeding that slowly gets lighter over several weeks
  • Soreness, cramping, and tiredness
  • Big emotions, including tears, in the first two weeks (often called the "baby blues")
  • Night sweats and changes in your breasts as milk comes in

When to call your midwife right away

  • Heavy bleeding that soaks a pad in an hour, or large clots
  • Fever, chills, or a wound that is red, hot, or leaking
  • A severe headache, vision changes, or swelling in the face or hands
  • Chest pain or trouble breathing (call 911)
  • Feeling hopeless, numb, or having thoughts of harming yourself or your baby

Building your postpartum village

  • Food: line up a meal train before birth
  • Rest: someone else handles laundry, dishes, and visitors
  • Feeding support: know who you will call for breastfeeding help
  • Mental health: save a number now, so you are not searching later. Postpartum Support International has a Texas chapter.
